Welcome to the forum. Several years ago you did a nice job replacing the hooks on a VMM plate of mine.
This is a late war adaptation of the common Confederate time fuse adapter. These types are shown in Chuck Jones' fuse book on page 42.
The spanner wrench concept was limited to the right wrench being on hand and one of the prongs not breaking off. This "screwdriver slotted" head eased that limitation as any straight piece of metal would suffice to screw this type in. They also come in the longer version of the CS apater for rifled shells.
