polyhedral segment shell cavity form names?

Been a little while since I've been here . Your new "home" is quite nice.

Here is my question: Does anyone know the form name for the Bormann based shell that breaks into 2 squares, 8 rectangles and 8 triangles (18 pieces in all).

The diamond form is the rhomboidal dodecahedron and the the polygon form is the polygonal dodecahedron, but what is the form of the third cavity type?.

Spent a couple of hours trying to find a picture of this shape and figured if I knew the name I could find one easier.

  I don't know of any Geometric name for that shape, because it is a combination of several geometric forms.
